How does it save time if the doctor doesn't read it until the time of consultation? Time given for consultation is taken up by doctor reading the notes.
Even more time is wasted because the questionare selection of responces does not address the patient's issues. Therefore the correct reason cannot not be given and you have to explain at consultation.
It's a very perfunctory format. It feels like if the reason you want to see the doctor isn't listed, it mustn't be important. Great if you are suffering depression.
